REQUESTING AN APPLICATION PACKAGE TO ADD AN ADDITIONAL STORE TO AN EXISTING VENDOR AGREEMENT
 

If you currently have a Vendor Agreement with WIC and wish to apply for authorization of a new store location under that Vendor Agreement, you may request an application package to add a store as specified below. Please be aware that the ownership of the new store location must be identical* to the one identified in the current Vendor Agreement. If the ownership is not identical, then you must apply as a new vendor for the new store location. You can print an application package directly from the website.


Requesting an add to master application package
Contact your Vendor Consultant directly or call (916) 928-8705 and request an application package for authorization of an additional store. Or, you may mail your request to:

When requesting the application package, please be specific as to:

  • Name of vendor ownership
  • Five digit Vendor Contract Identification Number (on your current Vendor Agreement)
  • Mailing address where application package should be sent
  • Name of contact person
  • Phone number


* Identical ownership means that the same individual, partners, Limited Liability Company, or Corporation that is the vendor ownership identified in your current Vendor Agreement is the same owner for the new store location.
